Vital Records

Effective January 1, 2005, all individuals requesting a certified copy of a record (Pursuant to RSA 5 C-16) must present positive identification, including, but not limited to, a driver's license, passport or other picture identification, or in the absence of acceptable picture identification shall complete the documentary evidence form.

 

To obtain a certified copy of your marriage certificate you can come in to the office during normal business hours and with proper picture identification you can apply for, and with a short wait, obtain a copy of a vital record certificate. The Town Office is located at 15 Mechanic Street.

 

Or, you can apply for vital records by mail. You must print and complete our Order Form, include positive identification, including, but not limited to, a PHOTOCOPY of your driver's license, a PHOTOCOPY of your passport, or a PHOTOCOPY of other picture identification, or in the absence of acceptable picture identification shall complete the form State of NH Documentation Evidence for individuals NOT possessing an acceptable picture identification. Enclose this with your check made payable to: Town of Alstead, along with a self-addressed stamped envelope and mail to the Alstead Town Clerk, PO Box 65, Alstead, NH 03602.

  • A fee of $15 is required by law in the State of NH for the search of the file for any one record.

  • A fee of $10 is required by law in the State of NH for each subsequent copy issued at the same time as the initial copy.

 

Vital records are considered private and confidential, with access limited to those individuals who have a "direct and tangible" interest in the record. The only exceptions are records prior to 1948, which are considered public.

 

Only the registrant, a member of his/her immediate family, or a representative, may obtain copies of vital records or someone with a "direct and tangible interest" such as an attorney. In order to determine if the "legal representative" may have access to the record, the direct and tangible interest must be listed in any written correspondence with a copy of such legal document being provided.
